Job Role
To offer early intervention and ongoing support to prevent individuals with multiple and complex needs from returning to rough sleeping following discharge from hospital.
To provide a high quality service to clients who are housed in accommodation who have complex needs individuals.
To support people to sustain their quality of life, develop support plans and engage various support agencies.
Provide advice for safeguarding and health and well-being on a wide range of issues including housing, welfare rights, personal finance, hoarding, tenancy management, mediation, life skills and the promotion of personal independence.
Act as the principle point of contact for casework within their accommodation and to co-ordinate solutions for drug and alcohol, domestic violence, and mental health issues
