A fantastic opportunity has arisen for a Band 7 Paediatric Nurse to contribute to the enhancement and expansion of the Adolescent and Transition service. This role offers a dynamic environment where you'll support young people aged 13 to 18 years, providing specialized care and guidance across both children's and adult services.
Role:
As a Band 7 Nurse, your primary focus will be to assess and address the health and wellbeing needs of adolescents, especially those with chronic, complex, or long-term conditions.
You'll collaborate closely with multidisciplinary teams to design and implement transition programs, ensuring seamless care as young people transition from paediatric to adult services.
Your responsibilities will span various hospital areas, from pediatric wards to outpatient clinics and emergency departments, providing holistic support to patients and their families.
We're seeking a motivated and dedicated individual with a background in paediatric nursing and experience in adolescent care.
