Sanctuary Personnel is currently looking for experienced Mental Health Nurses to work within South London as a Mental Health Nurse Community Practitioner.

*Location: *South London
*Salary: *£22 - £31 depending on experience and NHS band.
*Contract Type: *Locum, 3-month contract

*The Role:*

As a Community Practitioner you will promote and develop clinical leadership and clinical practice and be a positive role model to junior members of the team through the practice of evidence-based care. You will be expected to support professional development through clinical appraisal and supervision and contribute to the ongoing educational programme for staff, fostering a positive learning environment.

*Duties of the Community Practitioner:*
* Recognise and address concerns about the physical health needs of service users with or without long term conditions (e.g. diabetes, asthma etc.); escalating these concerns as necessary to senior members of the multi-disciplinary team.
* Reporting any incident or Serious Untoward Incidents that may take place in a community setting and provide reports and accurate records relating to the incident.
* Recognise and respond appropriately to “self-harm and suicide prevention” with particular regard to the risks in community settings, in line with Trust policies and training guidelines. Respond to urgent situations, emergencies or crises using protocols specific to the local community setting.
* Lead and participate in team, locality and Trust initiatives as required and promote the strategic aims and objectives of the service and other relevant quality improvement initiatives. Work closely with others in the LWN Alliance to identify the health needs of the practice population and support the development of services in response to those needs.
* Monitor the side effects of medication, utilising evidence-based tools. Nurses administering medication including IM do this in accordance with NMC guidelines and code of conduct and a good knowledge of medication used in a community setting and side effects.

*Requirements of the Community Practitioner*
* Degree or equivalent in Mental Health Nursing
* Experience working as a Mental Health Nurse
* NMC Registration
* Significant clinical experience
* Understanding of Care Co-ordination under the Care Programme Approach
