Job overview
This is a new role in the Trust, providing strategic leadership, including development of education and research in relation to enhanced care delivery across the Trust in line with the Trust’s vision for personalised integrated care services.
The role will have a specific focus on improving quality of care delivery for patients who require enhanced support whilst in hospital due to any cause of neurological impairment, this includes mental health, dementia and delirium. This includes building on current and developing key relationships and pathways across the hospital and other specialist teams.
Main duties of the job
- To support person-centred and relationship-centred enhanced care within Ashford & St Peter’s NHS Foundation Trust and work to improve the experience of carers/families who are impacted during hospital admission.
- Provide expert advice and lead in the development of Trust-wide policy and strategy regarding service delivery and development, including analysis of service. Identify practice improvements and develop educational and quality initiatives across all areas of bespoke care delivery to meet patient needs.
- Provide strategic leadership in enhanced care (including MH and dementia) delivery across the Trust in line with the Trust’s vision for personalised integrated care services with a specific focus on improving quality of direct care delivery.
- To manage own workload so that a proportion of time is spent providing direct autonomous professional practice; with the remaining time spent developing strategic services alongside existing Trust leadership. The working hours will be normal office hours but there may be an exceptional requirement to work outside these hours to ensure a clear understanding of the day-to-day issues affecting the Team both in and out of hours.
- To manage the specialist enhanced care and dementia team and ensure appropriate support and training is provided to the team
- The post-holder will play a key role in embedding the clinical framework for enhanced care across all staff groups.

Detailed job description and main responsibilities
General responsibilities
· Manage the Trust’s enhanced care and dementia team.
· To provide managerial and clinical supervision to the team.
· To ensure that the team is kept up to date with local and national best practice, and that regular briefings occur within the team to share this information.
· To ensure that effective communication systems operate within the team and between the team and other relevant disciplines and teams.
· To ensure local policies, procedures and protocols relevant to the health and well-being of the client group are maintained and developed if needed.
· To ensure the recording and investigation of all accidents, complaints, untoward incidents and losses relating to team activity are in accordance with Trust policy.
· To manage team sickness and absence according to Trust policy.
· To manage team performance, capability or disciplinary issues, as required, in line with Trust policy.
· To maintain up to date knowledge of resources, service provision, policy context, new approaches, interventions and treatments to support care planning and risk assessing for patients requiring the support of the team and to participate in local policy and procedure development and review.
Clinical or technical
· Be an expert resource and role model in influencing and leading on professional practice developments to improve clinical standards and care for people requiring enhanced care support.
· Role model expert clinical practice using a range of psychosocial interventions and emotional support to promote health and wellbeing for people with enhanced care needs.
· To lead on quality improvements in the field of enhanced care planning and delivery, including a patient-centred approach that supports development of professional practice and enhances the experience of people who require enhanced care provision whilst in the hospital.
· To be an innovator and use evidence-based practice to achieve continuous improvements in clinical practice.
· To make and support practitioners to make complex decisions in challenging circumstances.
· To work as an autonomous practitioner when needed, taking key responsibility and accountability for effective care delivery and planning.
· To provide clear leadership for the team, particularly around ensuring that the care of those with complex needs is supportive, therapeutic and improves patient care and patient experiences.
· To lead on the development of clinical audit and effectiveness in regard to the work of the enhanced care service, disseminating results of clinical audit and effectiveness to local, and where appropriate, national audiences.
Information and reporting
· To maintain the data collection spreadsheet and incorporate additional areas of data capture as required.
· To contribute to bi-monthly safeguarding committee reports and other committee/Trust reports as required.
· Ensure compliance with Trust policies, procedures and guidelines, taking the appropriate action if there are any concerns in relation to poor practice.
Professional, Communications and Engagement
· Provide professional leadership by developing, communicating, and implementing a strategy for delivering enhanced care for people with a variety of conditions which may be permanent or transient in nature.
· Be recognised as expert professional leader within Trust in relation to enhanced care, providing consultancy in enhanced care support, ensuring all advice and services provided are evidence-based and in line with national and local policies and guidelines.
· Work collaboratively with key stakeholders to ensure representation of the Trust at programme development meetings and be accountable for developing and influencing local policy and clinical practice.
· Contribute to the development of care programmes to enrich the care experience of patients with enhanced care needs.
· To work as a clinical leader, working as a role model across teams and departments to ensure that practice is evidence based, standards are met to evidence improved patient outcomes and improve satisfaction levels.
· To act as a change agent, and innovator; planning, instigating, and evaluating change as part of a cycle of continuous improvement.
Education, Training, Research
· Develop and lead the educational strategy and delivery in the field of enhanced care and add to the evidence base for people with enhanced care needs.
· Work collaboratively with Trust leadership team to influence the workforce development strategy to ensure skills, competencies and training needs are identified for workforce to support delivery of improved patient care, including participation in research programmes.
· Critically appraise outcomes of research in enhanced care services, using the findings to influence and support implementation of evidence based best practice within the Trust.
· Contribute as appropriate to pre and post registration higher education programmes.
· To lead, direct audit and evaluation of service quality using relevant standards supporting best practice.
· To develop a learning environment liaising with local researchers, practice developers and education providers.
Practice & Service Development & Evaluation
· Ensure that the service delivery model and future service needs are implemented in accordance with best practice and relevant local and national policies.

Qualifications and Further Training
Essential criteria
- Registered Mental Health Nurse (RMN)/ Registered Nurse with current registration. · Or Registered MH practitioner · Or other Registered Nurse/Registered Professional with demonstrable experience of working within a mental health care setting.
- Educated to master’s level in Health or a Management related subject or equivalent experience
Desirable criteria
- Other courses relevant to liaison psychiatry.
- Qualifications or equivalent experience in any of the following - counselling, cognitive behavioural therapy, psychosocial interventions or brief solution focused therapy.
