SPRING is a partnership of social sector organisations, commissioned by Northamptonshire Health & Care Partnership, to improve the wellbeing of people in Northamptonshire using Social Prescribing.

We take a person-centred approach and a holistic view towards wellbeing. Our goal is to help people to take a leap forward in managing their wellbeing and long-term health conditions.

About the role


We wish to recruit someone with experience of working directly in community development, adult health and social care, learning support or public health to join our expanding Social Prescribing team.


Social prescribing empowers people to take control of their health and wellbeing through referral to Spring Link Workers who take a holistic approach to an individual's health and well-being.

Spring Link Workers connect people to diverse community groups and statutory services for practical and emotional support.

The Spring Community Connector has the very important job of taking individuals by the hand through the journey of improving their wellbeing by taking practical steps.

You will support people as they learn how to navigate the community on their own.


You will motivate, encourage and support people to achieve the goals identified in their action plan by supporting them to attend a community group for the first time and less often supporting a person to access a service.

The Spring Community Connector will also support existing community groups to be accessible and sustainable, and help people to start new community groups, working collaboratively with local diverse partners.


The role will require managing and prioritising your own caseload, in accordance with the needs, priorities and any urgent support required by clients on the caseload.

It is vital that you have a strong awareness and understanding of when it is appropriate or necessary to refer people back to the Spring Link Worker.


About you
You must be able to listen and empathise with people and provide person-centred support in a non-judgemental way.

If you are able to get along with people from all backgrounds and communities, respecting lifestyles and diversity, this post would suit you.

A commitment to reducing health inequalities and proactively working to reach people from all communities is essential. As you will be visiting people in their own homes you will need to have your own transport.


Experience of supporting people, their families and carers in a related role (including unpaid work) is essential, as is experience of working with the VCSE sector in paid or unpaid capacity including with volunteers or small groups.

This can be a highly-rewarding job where no two days are the same.

If you have suitable experience and the confidence to work with GPs, PCNs and wider Multi-Disciplinary teams, and to work with commissioners and local partners to identify unmet needs within the community and gaps in community provision, we'd love to hear from you.

By supporting people with long-term conditions (including mental health issues), and people who are lonely or isolated, social prescribing can help to strengthen community resilience and reduce health inequalities.
