Consultant with experience in acute and/or adult general medicine required to develop a new Community Health Service across Oxfordshire


Our newly configured Intensive Community Care Pathway comprises a number of services which provide same day, acute and urgent care to patients as a safe alternative to secondary care and the Emergency Department.

These services support the delivery of high quality care closer to patient's own homes, allowing them to maintain their independence for longer and ultimately experience better personal outcomes.


The services include one of three Community SDECs placed in Witney, Abingdon and Henley treating a range of adults that are medically unwell and may require a range of treatments including iv infusions, antibiotics, blood transfusions, nebulisers, nursing, and therapy assessments.

As the Medical Lead for a single community SDEC unit, you will work alongside the operational and clinical management teams to oversee the provision of a high quality service, ongoing development and innovation of this unit with support from the Medical Director, and seek to integrate its functions with other services provided by Oxford Health and external system partners.

This is a hands on clinical role with a mix of educational and management responsibilities.

You will work collaboratively with the clinical leads for our Hospital at Home and Urgent Community Response teams.

Working together you will ensure commonality between units and support each other with workforce, learning and development to achieve our shared goal, and so that patients are fully cared for closer to their own home.


You will support the teams in understanding clinical risk and identifying decision making mechanisms that aim to treat the patient in their best interests.

The post holder will provide an agreed number of clinical sessions in one of the Community SDEC units with opportunity to work clinically in other settings both internally and externally depending on job planning.


The post holder will support the Medical Director and Managing Director to advise the Trust, and support and develop whole system innovation in community based care pathways.

This particular post is part of the Primary, Community and Dental Care Directorate in Oxfordshire and sits within the Integrated Intensive Community Care Pathway (ICC).

The post holder will work hands on clinically for part of their contracted hours, providing medical leadership, acting as a role model for all clinical staff in theinterdisciplinary team.

This role will be heavily involved in education; developing evidence based clinical pathways for the management of acute conditions commonly encountered in acute and urgent ambulatory care.

There will also be a strong interface with other stakeholders, including local acute hospitals and the ambulance service. The work will be varied and fast paced as you lead colleagues through a period of challenge and change.
