Job Description

  • Diabetes is a key priority in Barnet Commissioning Strategy Plan which is linked to the Diabetes National Service Framework. Barnet has developed a model of care for the management of patients with Diabetes and the role will support and help primary care teams to deliver this model of care.
  • The post holder will provide support to all practices and other services to develop a structured approach to the management of Diabetes, to work to agreed clinical protocols and guidelines ensuring comprehensive care across the diverse population of the Borough and to ensure patient self- care for their condition. The role includes managing an agreed caseload, education, training, facilitation, audit, working with the Practices to develop robust systems and help them to improve the care of poorly controlled diabetics. A team approach to implementing and delivering the service is paramount.
  • To provide core aspects of diabetes care management across appropriate health care settings i.e.
  • management of risk factors, provide basic dietary advice, treatment planning, education, screening for complications etc.
  • To act as an autonomous practitioner (often working in isolation), planning and implementing programmes of care for people with diabetes, their families, and carers.
  • Influence multi-disciplinary teamwork and communication between health social and voluntary organisations to deliver coordinated seamless patient care.
