Social Worker

Assess the needs of vulnerable adults and their carers. Together with the service users and carers, formulate and implement care plans appropriate to their needs.
Provide counselling and continuing support to service users.
Monitor, review and evaluate services in order to ensure that they service are provided effectively and in accordance with their need.
Participate in the operation of duty and respond accordingly to need.Provide an assessment service to individuals, their carers and other agencies in accordance with fair access to care eligibility criteria.
Initiate and co-ordinate care plans for and with individual's service users and their carers, in conjunction with professional colleagues and representatives of voluntary and private agencies.
