We are currently recruiting for 2 Specialist Targeted Support Workers on a 12 month Fixed Term contract:

Post 1: 20 hours a week - worked over 4 /5 days (Monday - Friday) Including early mornings 7am-9am and late afternoons/early evenings.**
Post 2: 16 hours a week - worked over Friday - Sunday**
Salary advertised will be calculated pro-rata.


As a part time Specialist Targeted Support Worker you will be providing 1-to-1 support to children, young people and families in their own homes and in the community.

These children and young people will be experiencing social, emotional and behavioural difficulties. You will have a caseload of clients, which will consist of mostly medium need, challenging & complex cases.

This is an exciting opportunity for individuals who have flexible hours and would like to work in a challenging yet rewarding role.

About the role


You will liaise with service managers, case Social Workers/Personal Advisors and the Commissioning team to agree a tailored targeted support package for each matched referral.

You will respond to Targeted Support Outreach referrals that come in as an emergency during the week or Friday afternoons that may turn into a longer piece of work.

For non emergencies, you may attend some introductory meetings to agree SMART goals and work with clients to achieve these & record/evidence contact for each session.

You Will complete risk and needs assessments and raise all safeguarding concerns as needed.


You will make referrals to other AfC services and external organisations for additional support and contribute to relevant case meetings.

Types of support could include respite for carers, unannounced home visits, supervised contact, escorting children to and from school and helping manage family relationships, routines and behaviour in the home.

You could also be required to be an appropriate adult.

About you

  • Educated to GCSE level with a relevant professional qualification in working with children or young people, youth work or supported housing
  • Experienced in working with families with complex needs e.g. mental health & substance misuse
  • Experienced in working with children & young people who display challenging behaviour and experience managing & reporting safeguarding incidents
  • Experienced in working alone in the community and holding strong personal boundaries
  • Advocating the voices of those we are working with.
  • Experience of keeping and maintaining accurate records (in accordance with HOST policy)
  • Able to respond to emergency situations with limited information and risk assess appropriately
  • Experience in inspiring and encouraging young people to engage in positive activities
  • Support young people to attend all agreed appointments and act as an appropriate adult
  • Ability to work some unsociable hours including early mornings, evenings and weekends
  • Able to promote and model healthy and positive relationships with families and young people in their support networks
