About the role

We have an exciting opportunity to join our Liaison and Diversion Team as a support worker, based at our Durham Service.

Our aim is to improve the lives of people who come into contact with the criminal justice system and have learning or mental health needs, or who are experiencing other vulnerabilities.


  • The role includes sign posting and identifying ways you can support the people you work with to help them to make a difference. You will be working in a multidisciplinary team so you will be able to give the very best support whilst enhancing your own practice.

  • Support Work is a diverse and rewarding role, key duties you will undertake are:
  • To work within a multi-disciplinary team to provide information, advice and guidance, referral and direct individual support to enable access to support services for adults, children and young people identified through custody based triage as having mental ill health, learning disability or substance misuse issues to promote mental and physical wellbeing.
  • To deliver time limited support interventions to adults, children and young people to promote engagement within wider support services.
  • To appropriately engage individuals with peer support and promote engagement with those with lived experience.
  • To develop and maintain strong links with partners and stakeholders such as police, court, probation services, mental health teams, substance misuse services and learning disability providers alongside a range of health and wellbeing services.
  • To support ongoing development of the Liaison and Diversion service within both the criminal justice system and wider health and social care networks.

    Who will you be working with.
You will be working as part of our Liaison and Diversion Team.

The team is a multi-disciplinary criminal justice team comprising of colleagues from Humankind, Spectrum and Tees, Esk and Wear Valley NHS.

The team includes Nurses, Mental Health Nurses, Navigators, Support Workers, Peer Apprentices and Volunteers and specialist workers.

We are a supportive and welcoming team, who are passionate about supporting and promoting recovery for those we care for and within our local community.
