Our Renal Department are looking for an efficient, friendlyPatient Pathway Coordinatorto facilitate the smooth running of the patient journey by ensuring that coordinated and streamlined administrative processes revolves around the patient and their individual needs.


Your role will be to provide administrative support to all clinical staff within the clinical area, including Consultants, Nurse Specialists and Junior Doctors, ensuring effective co-ordination of their diaries etc.

this will include cancellation and reinstating of clinics in line with planned and unplanned leave and ensuring a consistent and effective administration service is maintained at all times.


You will support the clinical team, Operational Manager and other Divisional managers in ensuring the specialty are able to deliver safe and effective services whilst also ensuring compliance with contractual access targets e.g., the 6-hour ambulatory target and CQUIN compliance.

Good telephone communication skills are essential, especially dealing with patients, district nurses and outside agencies.


Your role will be to co-ordinate, monitor and initiate appropriate changes within administrative processes along the patient pathway to ensure the patient's journey is managed efficiently, smoothly and in accordance with targets and agreed timescales, liaising with teams outside of the specialty e.g., GPs where appropriate to ensure a seamless patient journey.


You will utilise computerised tracking systems to locate case notes and investigation results, ensure patient information is maintained and updated on the appropriate patient information system, monitor work lists to monitor Consultant workflow and escalate to Operational Managers if necessary and patient attendance to appointments and re-arrange/book further appointments.

Knowledge of TCI's and pre assessments would be useful.
