The Care Navigator is the effective link between the patient and services provided.

The Care Navigator should liaise and work with the Practice Team and other Healthcare Professionals and is expected to be proactive in maintaining high standards of patient services.


Job Purpose:

To work within General Practice to help implement the "Putting Patients First" Model of care to support patients to improve and/or maintain health and wellbeing

To help signpost patients across health, social and community services

To encourage and support patients to self-manage through coaching and endorsing self-care approaches, signposting and using IT systems to facilitate

To attend training as appropriate to maintain competence in the role

Main Duties & Responsibilities:

To manage patient queries by phone or in person and signposting effectively following reception rules and procedures

To use the electronic clinical records system to correctly document patient appointments, messages, requests, contact details and correspondence

To encourage excellent communication, when meeting and greeting patients and visitors in person and over the telephone

To inform patients of any delays or cancellation of appointments

To receive samples from patients for processing using the correct protocol

To ensure the reception and waiting areas are kept clean and tidy

Job Types:
Full-time, Permanent

Salary:
£10.00 per hour

  • Brigg: reliably commute or plan to relocate before starting work (required)

Education:

  • GCSE or equivalent (required)

Experience:

  • Training: 5 years (preferred)
  • Health and
Social Care: 3 years (preferred)
