Job Title: Advanced Clinical Practitioner (ACP) for Older People and Frailty at Whittington Health

Join our Acute Frailty Team as an ACP and play a vital role in providing expert assessment and care for frail elderly patients. Your responsibilities will include coordinating quality improvement initiatives, educational sessions, and staying updated on the latest guidance for care of older people with frailty.

Main Duties:

  • Review and assess frail elderly patients
  • Liaise with multidisciplinary team members
  • Communicate with patients and families
  • Provide patient advocacy
  • Deliver patient education and instruction
  • Identify and refer patients to additional services
  • Lead and support the multidisciplinary team
  • Initiate clinical practice improvements
  • Identify and resolve patient-related issues

Education:

Identify education needs, develop training strategies, and lead education programs. Encourage research-based practice and provide support for staff knowledge development.

Leadership, Management, and Service Improvement:

Champion patient-centered care for frail older people. Contribute to service improvement strategies, support dementia care initiatives, and lead by example as a clinical leader. Empower staff, promote innovative problem-solving, and oversee the development of specialized care wards.

Research and Audit:

Participate in audit programs, support ongoing service audits, evaluate research findings, and ensure service effectiveness.

Person Specification:

Clinical Skills - Essential Criteria:

  • At least 2 years experience working as an ACP in an acute setting

Education/Qualifications - Essential Criteria:

  • Full NMC registration
  • Postgraduate level qualification in Advanced Clinical Practice

Communication Skills - Essential Criteria:

  • Effective communication skills
  • Works in partnership with patients
  • Supports patients in accessing necessary information for decision-making

Safeguarding Children and Adults:

  • Comply with Safeguarding Children and Adults policies and protocols
  • Take part in required training and supervision
  • Maintain professional boundaries and adhere to codes of conduct
