Job summary:

The Specialist Rehabilitation Pathway Co-ordinator post is an exciting opportunity for an experienced clinician to join our team.


The post is ideally suited to an experienced clinician who is proactive and self-directed and an effective communicator and decision maker.

Individuals should be able to work autonomously and creatively in problem resolution and capacity management.

They will liaise and negotiate extensively with internal and external agencies and build relationships to optimise patient flow and reduce delays in transfer of care.


This post offers an excellent opportunity for an experienced clinician to further their knowledge of rehabilitation pathways and commissioning to support career growth and development.

The successful applicant will be supported by an established team including Consultants in Rehabilitation Medicine, Therapists and Nurses who have a wealth of experience in delivering high quality clinical care and rehabilitation to this complex patient group.


Main duties, tasks & skills required:

As the Specialist Rehabilitation Pathway Co-ordinator you will work as part of the SHARP Team with a defined caseload of patients who have complex rehabilitation needs - these needs are primarily but not exclusively neurological in presentation.

The majority of patients are within our regional neurosurgical and neurology beds but the SHARP team also provides specialist input to patients who have specialist rehabilitation needs across all clinical specialities within QEHB - this includes critical care, major trauma and other regional and local specialty beds.


Working with the Multi-Disciplinary Team (MDT) you will use your clinical expertise and sound understanding of different pathway options to proactively manage the patient journey and facilitate patient discharge into a range of specialist rehabilitation settings or directly into the community.

In addition to the MDT, you will regularly communicate with the patient, their next of kin, internal systems and external providers/agencies to ensure safe, efficient and timely transfer of care.


In this role you will have profession specific support in addition to multi-professional support, with further professional development opportunities within the Trust and through attendance at internal/external courses.

The Therapy department has a strong emphasis on personal development with links to the School of Nursing, AHPs & Midwifery, and several Universities across the region.

Qualifications:

Essential:

Appropriate professional qualification e.g BSc, or MSc

HCPC registration Professional Body registration

Evidence of commitment to advanced clinical and non-clinical continued professional development within own speciality, e.g. post registration training and evidence of a detailed personal development portfolio

Experience:

Essential:

Significant post registration experience.

Experience of staff management including supervision and of development and performance management of staff

Significant experience of working in a multiprofessional team

Experience of clinical/service innovation and evaluation

Proficient knowledge in the specialist assessment and treatment of a wide range of complex acute and chronic clinical presentations within this specialty

Expert clinical knowledge of complex clinical conditions and rehabilitative requirements

Expert knowledge of setting, implementing and monitoring a realistic case management plan within a comp
