Humber Teaching NHS Foundation Trust's Award winning YOURhealth service are looking for an enthusiastic, dedicated individual who is passionate about helping and supporting people to become healthier and change their lifestyle as a Health and Wellbeing Connector and Advisor within the Health and Wellbeing, Connector and Advisor Service.
The post advertised will be to work with our existing team of dedicated Social Prescribers in several locations in the East Riding and York District, namely Bridlington, Goole, Strensall, Stamford Bridge.
This particular role will be based in Goole and the surrounding villages but may require travel to other locations within the East Riding.
Health and Wellbeing Advisor and Connectors give one-to-one and group support, help people to overcome barriers to improve their lifestyle and provide on-going encouragement and motivation.
