We are looking for a Full-Time prescription clerk to work either 4 or 5 days per week.

JOB SUMMARY


To act as a focal point of communication between the GPs, Clinicians, Medicines Management Team (MMT), Administrative Staff, Prescribing Team, Patients and local Pharmacists, as well as other members of the primary health care team, to ensure patients' repeat medication is produced within the current protocols of the surgery.


To support the Pharmacy Technician and Clinical Pharmacist(s) in ensuring the delivery of safe, effective and efficient systems for repeat prescribing, medicines optimisation, reducing medicines waste and maximising patient outcomes.


JOB DESCRIPTIONS

  • To have knowledge of and adherence to the practice's repeat prescribing protocol and to ensure repeat medication is issued in line with existing protocols and ensure that any queries raised are dealt with quickly and correctly.
  • Produce prescriptions for the GP's (or nonmedical prescribers) to sign in an acceptable timeframe.
  • Ensure all prescription requests are processed and are ready for collection within a 48hour timeframe.
  • Within guidelines and according to the Practice's protocols on prescribing, generate requests for medication on behalf of the patient for nonrepeat items within guidelines.
  • To assist in training administrative team members, relevant to the individual job role, and when revisions are introduced.
  • To liaise with patients and pharmacies regarding queries and requests and ensure followup action is taken if the enquiry cannot be resolved immediately.
  • To ensure that patient queries on medication are highlighted to the Medicines Management Team or GP.
  • Actively encourage patients to reconcile medications to facilitate monthly collections.
  • Discuss with patients their requirements, encouraging patients to order only what they require.
  • Act upon compliance issues, liaising with the Medicines Management Team or appropriate clinician.
  • Ensure medication reviews are arranged by maintain an accurate recall system.
  • Effectively liaise with external services i.e. district nurses to ensure medications are arranged for housebound patients.
  • Input data into the patient's healthcare records as necessary and within guidelines.
  • Deal with all prescription related enquiries in a timely manner.
  • Record and reported adverse effects accurately and appropriately, informing the clinical pharmacists or GP.
  • To attend administrative meetings, prescribing meetings and PLT sessions.
  • Read code data on Emis Web system
  • Carry out audits as requested and within the time frame agreed.
  • Uploading bloods onto a patient's record and book for blood test / BP / weight / height when needed.
  • Add blood tests from DART / clinical letter onto a patient's record.
  • Uploading clinical / nonclinical letters once relevant training has been signed off by MMT
  • Action midwife requests once relevant training has been signed off by MMT.
  • Action LRU (Local Referral Unit) requests once relevant training has been signed off by MMT.
  • Complete a Near Miss log sheet when appropriate and pass to MMT for review
  • Maintain a clean, tidy, effective working area at all times.


Duties may vary from time to time dependant on current and evolving practice needs but with consultation with the post holder.

  • Taking messages and passing on information as appropriate.
  • Computer data entry/data allocation and collation, processing and recording information in accordance with practice procedures.
  • Initiating contact with and responding to requests from patients, team members and associated healthcare professionals and providers.
  • In accordance with the practice's Health and Safety Policy, helping keep all practice areas tidy and free from obstruction and clutter.
  • Helping to promote and maintain a positive, supportive culture across the whole of the practice team.
  • Any other duties as reasonably requested by the partners/management and any duties commensurate with this position.

  • Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development.
