Patient Pathway Coordinator Band 4


We are recruiting to a new Smoking Cessation Service, which will be pivotal in helping our patients give up smoking and/or the use of other tobacco containing products through evidence-based interventions.

The NHS Long term Plan commits to support every person admitted to hospital who smokes by offering tobacco dependence treatment.

This includes expectant mothers throughout antenatal care.

Smoking is one of the leading causes of preventable illness and death, and there is evidence from Ottawa and Manchester to suggest that implementing a hospital-based tobacco dependence team can contribute to significantly improved health outcomes for patients and communities.

These include reduced morbidity and mortality from smoking and tobacco dependence, improved medical and surgical outcomes, reduced hospitalisations and readmission rates, and addressing/reducing the health inequalities associated with higher rates of smoking in more deprived populations groups and areas.

  • Record all referrals to the service and ensure referrals are acting upon by the advisory team
  • Support and facilitate the advisory team by ensuring all people referred to the service are assessed, reviewed and discharged within the appropriate timescales.
  • Coordinate, administrate and attend team referral meetings
  • Maintain clinic lists for initial assessments, follow up appointments and virtual consultations,
  • Investigate "did not attend" or disengagement, communicating the results to the Advisory team,
  • Cancellations communicate details to the ensuring all information is available
  • Use Trust IT systems and specialty databases to ensure relevant and accurate information is recorded.
  • Ensure healthcare records are tracked to the correct location,
  • Coordinate, administrate and attend team meetings to ensure team functionality and communication
  • Administrate by ensuring all agenda items, action plans and minutes are true and correct and available at time of meetings.
  • Provide cross cover within admin team, deputising as required.
  • To carry out duties consistent with the post as required by the Patient Pathway Coordinator
  • Responsibility for location, collection and management of required key data items required for reporting
  • Develop and run reports to monitor the performance of the Smoking Cessation service
  • Be the point of contact for informal complaints
  • Support the Lead Specialist in service development projects
  • Support work flow management and improve efficiency of the team
