Job summary

Mirfield Centre Centre is seeking an Advanced Clinical Practitioner to join our supportive team of GPs, Advanced Clinical Practitioners, Pharmacists and Nurses.

Main duties of the job

TheAdvanced Clinical Practitioner(ACP) has been assessed as competent inpractice using their expert clinical knowledge and skills. They have thefreedom and authority to act and make autonomous decisions in the assessment,diagnosis and treatment of patients.

The postholder works at the level of advanced clinical practice and has developed andcan evidence the underpinning competencies applicable to the specialty orsubject area, , the knowledge, skills and behaviours appropriate to the healthcareprofessionals (HCP) setting and job role.

The roleprovidesan opportunity for patients to avoid the need to be referred to a GP, therebyenabling them to receive timely care and negate unnecessary delay in receivingtreatment.

An ACP is autonomous in making decisions basedon assessment, diagnosis and interpretation of test results and will initiate,evaluate and modify a range of interventions which mayinclude prescribing medicines, therapies, lifestyle advice and care.

Clinicalpractice

Assess,diagnose, plan, implement and evaluate treatment/interventions and care forpatients presenting with an undifferentiated diagnosis.

Clinicallyexamine and assess patient needs from a physiological and psychologicalperspective, and plan clinical care accordingly.

Assess,diagnose, plan, implement and evaluate interventions/treatments for patientswith complex needs.

Proactivelyidentify, diagnose and manage treatment plans for patients at risk ofdeveloping a long-term condition (as appropriate).

Diagnoseand manage both acute and chronic conditions, integrating both drug- andnon-drug-based treatment methods into a management plan.

Prescribeand review medication for therapeutic effectiveness, appropriate to patientneeds and in accordance with evidence-based practice and national and practiceprotocols, and within scope of practice.

Workwith patients in order to support compliance with and adherence to prescribedtreatments.

Provideinformation and advice on prescribed or over-the-counter medication onmedication regimens, side-effects and interactions.

Prioritisehealth problems and intervene appropriately to assist the patient in complex,urgent or emergency situations, including initiation of effective emergencycare.
