The National Discharge Frontrunner Programme Manager will lead the transformation programme of work for discharge and wider admission avoidance schemes across Humber & North Yorkshire ICB.


As a national frontrunner you will lead a programme that is one of six nationally and be expected to work at the most senior level in the ICB and with wider senior NHS leaders.


The National Frontrunner transformation programme will lead the way in developing and testing radical new approaches to post-acute care that sees patients discharged to the right place, and with the right support, in a safe and timely manner.


At the heart of this new programme of work will be community digital transformation, working with system stakeholders to design, develop and test new digital solutions that support intelligent decision making through the use of wider system dashboards and data.
