Job responsibilities

Beprimarily responsible for the rapid assessment and treatment of patients whohave urgent and/or complex physical / mental health and social care.

Providehealth care advice and education to team members, statutory, voluntary, andindependent sector providers

Beresponsible for health promotion, teaching and training of support workers,pre-registration students, patients, relatives, and carers

Ensureup to date written and electronic records and activity data are maintained inaccordance with professional and trust standards

Prioritisecaseload, with professional demands, considering the needs of other members ofthe team.

Community experience Experience discharging planning of patients from hospitals Experience of caseload management and prioritisation
