The Post COVID Assessment and Support Service (PCASS) provides assessment and rehabilitation support to patients living with long COVID.

Role provides prompt, efficient co-ordination and administration support to the PCASS multi-disciplinary team (MDT) within the health setting.

Provides supervision and support to the team of care coordinators.


Works with relevant MDT professionals to support the assessment and care provision processes including tracking progress and conducting patient follow ups and running group sessions.


Ensures prompt response to referrals and co-ordinates interventions by team members or ensures that referrals are made promptly to other service.


Maintains the information systems used by the team in order to collate and co-ordinate performance data and produce reports as required for the team and relevant manager.

The service provides equitable access to assessment and support for people experiencing Post COVID-19 Syndrome

This includes:
Multidisciplinary assessment with consideration of physical, cognitive and psychological needs.

An enhanced assessment and support offer including supported self-management and care coordination to help patients navigate their recovery journey.

Innovative treatment and rehabilitation pathways for patients whose needs fall outside of existing community pathways.

To provide supervision and support to the team of care coordinators.


To be the point of contact for patients and their families/carers where information is required about the support and care provided e.g., anticipated visits, the actions of team members.

To sign-post referrals to team members and other services as appropriate.


To process and administer referrals made according to the relevant health criteria seeking advice from relevant team members when required.

To request completion of an initial holistic assessment by a relevant MDT team member.

To monitor, track and report on progress within the patients journey via consultations.

To work alongside colleagues to lead on self-management groups for patients with long COVID symptoms.


To provide a client centred approach to ensure all clients' and carers' views and opinions are considered and all people who make an enquiry are given respect, dignity and understanding.

To escalate any issues or delays to relevant Team Lead for appropriate action.
