To assist and support the Mental Health and Dementia Practitioner in working collaboratively across the MDT to increase access and reduce health inequalities for people diagnosed with mental health condition and or dementia who use services at the Royal Orthopaedic Hospital (ROH). Main duties of the job
  • Strong organisational skills and the ability to analyse situations and be able to make the right decisions.
  • Able to empathise, be supportive and sensitive to the needs of others as well as being able to cope with distressing circumstances.
  • Ability to cope in a pressurised environment and apply practical problem-solving skills in everyday situations.
  • Competent IT skills and the ability to navigate around various systems and software packages (such as outlook, databases, MS office and the internet)
  • Ability to work effectively in a team, work on own initiative, work unsupervised and manage own workload.
  • Manage difficult situations with service users that have complex mental health conditions and dementia.
  • Promote effective liaison with all statutory and voluntary agencies concerned with dementia and mental health.
  • Working knowledge of the complex needs of people with dementia.

  • To assist with care planning for patients who are diagnosed with a mental health /dementia condition
  • To provide support to the patients carers, ensuring reasonable adjustments are in place to support the patients needs.
  • To work in collaboration with the MDT to develop plans of care for both outpatient services and elective admissions ensuring the provision of safe, high-quality services.
  • To support the Band 7 Mental Health and Dementia Practitioner to provide specialist knowledge to support patients to reduce health inequalities.
  • To support and de-escalate crisis when referral received from ward.
  • Assess and support patients whose psychological distress is because of social and or environmental factors.
  • Signpost patients to more appropriate services including support around the catalyst of poor mental health that might include a social crisis issue.
  • Assist the Mental Health and Dementia Practitioner with the implementation of Government papers and policies.
  • To be actively involved with professional/ nursing groups within the Trust and externally as requested by the Mental Health and Dementia Practitioner
  • Be an advocate for all patients within the Trust under the mental health and Dementia Service.
  • Provide timely and accurate statistical information as required. Produce comprehensive, accurate and up to date patient records
  • To have an awareness and understanding of the safeguarding risks associated with people who have a mental health condition and or dementia.

Person Specification Qualifications Essential
  • Current entry on the NMC Register - Mental Health , RN, Or HCPC, Social Work England
  • Evidence of continuing professional development
Desirable
  • Evidence of further training/ CPD in Mental Health and or Dementia
  • In-depth knowledge of Safeguarding Vulnerable Adults and Child Protection and Mental Capacity Act and DOLs/ LPS
Experience Essential
  • Substantial experience within health or social care setting relevant to role
  • Experience of working in a Hospital enviroment
Desirable
  • Experience of working within a mental health setting
  • Experience of working with a person living with dementia
  • Experience of handling clinical incidents and complaints
Skills and Knowledge Essential
  • Demonstrate a knowledge of duties required for the post
  • Sound decision making,the ability to analyse situations and be able to make decisions.
  • Understanding of professional and current issues in nursing patients with Mental Health Condition and or Dementia
  • Awareness of equality and diversity
  • Able to work across professional teams and organisational boundaries
  • Excellent verbal and written communication skills
