Acute Medicine Advanced Clinical Practitioner

To lead and coordinate the delivery of care and development of SDEC service through the use of effective evidence based practice in line with agreed national standards for patient safety and clinical practice.
- To provide the highest standard of holistic care for patients through the utilisation of advanced clinical skills in the assessment and management of patients, working alongside the multidisciplinary team to improve the quality and safety of patient care.
- To work autonomously with mínimal/remote supervision and be accountable for the planning and delivery of all aspects of patient care, including medication prescribing in SDEC as set out in the relevant professional code.
- Lead the provision of specialist clinical knowledge and advice to other staff, the wider multiprofessional team, allied health professionals, patients and their families/carers and partner organisations.
- Lead the provision of education and training for other practitioners and allied health professionals; including health promotion
- The post holder will take a lead role within a team who provides high quality specialist clinical care.
Based in the North East of England we provide a range of hospital and community health services from our leading facilities, including the Queen Elizabeth Hospital, Blaydon urgent care centre and Bensham Hospital, all within Gateshead.

- To continuously improve clinical care standards in line with the clinical governance framework to ensure the highest quality of care is provide in the safest manner.
- To provide clinical leadership, expertise and specialist clinical advice and support to clinical staff on the management and care of patients within SDEC and lead the effective delivery of care to patients.
- To lead the provision of expert specialist knowledge, care and experience to support the wider multidisciplinary team to facilitate the effective delivery of care to patients at all times.
- To lead and participate in the continuing improvement of services in line with the Trust objectives.
- To lead in the provision of education and training to clinical staff/ T-ACP/Nurse Practitioners within SDEC and internal/external agencies.
Support the delivery of Trust VTE clinic in the absence of the Lead ACP, making clinical decisions on patient's treatment and management.
