Principal Tasks

  • Engages with and follows individual residents care plans ensuring individual, personalised support is provided in line with individuals needs
  • Updates residents care plans and risk assessments with accurate relevant information in a timely manner to ensure personcentred approach focussed on the individual's changing needs adapting practice accordingly
  • Use The Maples templates and processes to capture information and maintain residents files
  • Offers and presents options to support and empower service user choice reflecting the importance of appropriate risk taking in line with care plan and goals
  • Responsible for the capture of outcome information and data, using The Maples documentation and processes e.g. daily records, keyworker reports, to support the review process, discussions with commissioners and with families/carers
  • Complete specific, delegated tasks to deadlines and quality standards in line with The Maples Policies and procedures, reporting back to the senior, care supervisor and/or Home Manager as agreed.
  • Support residents within the home who need support and assistance with their mobility, and those with no mobility who need to be hoisted or transferred
  • Support residents with their daily personal care routine and continence and ensuring their dignity at all times.
  • Support residents with their medication, when appropriate training and guidance has been provided.
  • Provides Key worker support, regularly reviews residents information in line with The Maples policy and provides information to the individual, their significant others and external care professionals
  • Act as an advocate for residents during formal meetings or situations as required
  • Work in partnership with colleagues, appropriate family, friends, other significant to the individual and care professionals to support each individual service user.
  • Deal with issues, comments and complaints about service delivery in line with policy and process
  • Ensure residents are safeguarded by working within internal policies, medication management systems, staff handbook, all relevant external regulatory requirements (CQC etc)
  • Responsible for your own, service user and home visitors health and safety (H&S) at all times.
  • Works within the Maples policies and procedures and relevant regulations at all times

Self

  • Work in a way that reflects The Maples Vision, Mission & Values and promotes best practice
  • Uses initiative, does what is needed without being asked, takes accountability for self and the home
  • Attends 1:1, team and other external meetings in line with the role as requested
  • Adhere to working hours required, demonstrates flexibility to ensure the home is covered
  • Take personal development seriously, identifies development needs, works with line management to support this

People

  • Actively participates in team and other meetings attended, sharing relevant information
  • Reports concerns and important matters to line management in a timely manner
  • Presents the home and organisation in a positive and professional manner at all times
  • Actively participates in the supervision and annual individual review process
  • Supports new team members

Change

  • Supports and manages change proactively within the service, raising concerns appropriately

Job Types:
Full-time, Part-time

Part-time hours: 36 per week

Salary:
From £9.50 per hour

All interested staff are required by law to have at least their first COVID vaccination and will have had their second vaccination by the 11th November 2021.

Staff required to wear a surgical mask for their entire shift inline with government guidelines
