About the Role

Leighton-Linslade Health Connections has a fantastic opportunity for an Advanced Practitioner with Mental Health Specialist Interest to join our team on a full-time basis, for hours per week.

The post holder will work to a high level of personal and professional autonomy, using specialist knowledge and skills to deliver independent assessment, clinical examination, diagnosis, clinical decision making and planning of care and treatment, health promotion and safe admission and discharge of patients.

You will practise in compliance with your respective code of professional conduct and within your scope of practice, being responsible and accountable for your decisions, actions, and omissions at this level of practice.

You will demonstrate a critical understanding of your broadened level of responsibility and autonomy and the limits of your competence and professional scope of practice, including when working with complexity, risk, uncertainty, and incomplete information.

You will act on professional judgement about when to seek help, demonstrating critical reflection on own practice, self-awareness, emotional intelligence, and openness to change.

About the Candidate All applicants should have the following essential qualifications, registrations, skills and experience: A degree in nursing or qualified via registered nurse degree apprenticeship Holds a recognised membership of a relevant professional body Holds a post graduate qualification at level 7 or above relevant to mental health practice Two years' experience in post graduate work in core profession following qualification Two years' working in mental health services Two years working as an independent prescriber To be able to demonstrate application of therapeutic and analytical skills A clear understanding of child protection policy and procedures and commitment to safeguarding Ability to listen, empathise with people and provide person centred support in a non-judgemental way Commitment to reducing health inequalities and proactively working to reach people from all communities Ability to use own initiative, discretion, and sensitivity Ability to work on own initiative Ability to communicate complex and sensitive information effectively with people at all levels by telephone, email, and face to face Ability to identify risk and assess/manage risk when working with individuals High levels of integrity and loyalty. All applicants should have the following desirable qualifications, registrations, skills and experience: Recognised qualification in independent prescribing Collaborative multi-disciplinary working for two years Previously worked in a primary care setting or a PCN.

We offer various primary care support roles to help the population take ownership of their health and connect with the community; working with the Personalised Care Model to help patients achieve their goals and focusing on what is important to them.
