Job Overview
An exciting opportunity has opened up in our Selby Community Response Team for a dedicated and experienced Specialist Occupational Therapist to join us.
- This role involves providing holistic assessments and specialized occupational therapy to patients in their homes as part of a collaborative team.
- We work closely with various partners to support patients in their recovery and prevent unnecessary hospital admissions.

Main Duties of the Job
- Conduct comprehensive assessments of patients with diverse conditions.
- Develop and implement individualized treatment plans based on evidence-based practice.
- Ensure holistic care for patients in the intermediate care and fast response service.
- Manage own caseload and adjust plans as needed.
- Identify and report work-related risks.

Education
- HCPC Registered Occupational Therapist
Experience and Knowledge Required
- Minimum 2 years experience in a relevant role
