We find ourselves with an exciting opportunity to offer a 12 month contract working as part of the multi-disciplinary team, driving the changes to community care response to support the hospital avoidance agenda.

The post holder may have the opportunity to work across the Enablement and Urgent Care services, including A&E Therapy Services, Community IV Therapy Service and Intermediate Care bed base.


The aim of the team is to reduce unnecessary hospital admissions and prevent avoidable Emergency Department attendances by providing holistic multidisciplinary intervention and person centred approach support in order to stabilise patients in their own home or usual residence.

The service is provided across 7 days a week covering core hours of 8 am 8 pm. Please note working hours are under review with the possibility of extending the working hours 8am - 10pm

The post holder will be an active member of Urgent Care.

You will work with the Multi-Disciplinary Team to assist registered nurses and therapists in a range of patient care activities, assisting patients with activities of living, to promote faster recovery from illness and maximise independent living by providing therapeutic patient care as directed by registered staff.

You will be responsible for providing support work to aid the rehabilitation of patients assisting in the treatment of patients, in a variety of settings, e.g.

patients' homes and residential homes, ensuring appropriate pathways of care and communication via liaison with other agencies as required. This will involve lone working in the community.


You will support the initial assessment of patients who require the provision of aids, treatment on discharge from hospital or step up form community within own scope of practice.

Undertaking the treatment of patients within the speciality who may have complex acute or chronic presentations, some of which may not be straightforward.

You will be expected to support the falls lifting initiative to safely assess, lift and support patients who have fallen in their own homes.

Vital signs and Venepuncture would also be a beneficial skill to possess. You may be required to carry out a range of non-clinical duties if appropriate, e.g. housekeeping and clerical.
