Successful applicants will be invited to attend an Assessment Centre at Carew House, Bodmin on 17thFebruary 2023. This will be a morning or afternoon to be confirmed.


The role is similar to a Health Care Assistant in that you will be delivering great care and supporting reablement for people in mainly in their own homes.

Consideration for full and part time workers.


You will be based initially in Falmouth Health Centre and other bases in Truro or Trevissome can be considered, depending on your preference.

Travel costs for journeys during work time are supported and pool cars are also available for work use.


You will need to demonstrate good communication skills and a positive approach to people who may have communication difficulties or who have challenging health needs.


You will be part of a dedicated team and will be provided with bespoke training to support your development in this role.

You will be required to work a rota that covers 7 days a week from 08:00 to 20:00 hours. Flexibility within this rota, and geographical location, will be important.

This is a lone worker post.

  • The main purpose for this post is to ensure that opportunities for patients' reablement are optimised within a community setting.
  • The post holder will be rostered with other Community Support Workers to provide a 7 day a week service to community patients requiring reablement either before or after a hospital admission.
  • The Reablement workers within each locality will form a part of an Integrated Care Pathway for frail patients that involve a range of services.
  • This post holder will be expected to work with patients in the community, i.e. Care home setting and in patients own homes.
