An exciting opportunity has arisen for a specialised Band 6 Medicines Management Facilitator to join Neath Port Talbot Medicines Management Team for Domiciliary care.


The successful applicant will be part of the integrated, multidisciplinary Community Resource Team who provide support and education in medicines management, specifically in domiciliary and intermediate care settings.

This role aims to improve patient outcomes and minimising harm, including avoiding unscheduled care, admissions, and delays to transfers of care.

It is ideal for individuals interested in integrated working and are committed to developing and expanding their skills to achieve the highest quality care and outcomes for individuals in their own homes.


This innovative role recognises the contribution an enthusiastic and adaptable Pharmacy Technician can make to patient outcomes and the delivery of frontline domiciliary care services.

Excellent communication, change management and leadership skills will be essential.

The post holder must be able to travel between patient's homes and various sites within SBUHB efficiently.

The role includes:

  • Managing a defined caseload within the speciality, using evidence based/patient centred principles to assess, plan, implement and evaluate interventions, maintaining records.
  • Providing a lead contact between community pharmacy and social services with respect to medicines management issues, including the provision of regular updates, training and visits to community pharmacies to support the Medicines Administration Record scheme.
  • Working closely with and supporting the Community Medicines Management team lead, deputising during their absence including taking operational responsibility for the caseload, provide clinical leadership and coordinate activity for the Community Medicines Management team.
  • Contributing to the development of strategies and improvements in medicines management in the domiciliary care setting, to aid in sustainability, improve safety, quality and cost effectiveness and reduce waste.
  • The post holder is required to be a clinical expert in medicines management in the domiciliary setting, providing advice and information to multidisciplinary teams, service users, families and carers.
  • Undertake lone working as required.

Ensure the use of evidenced based practice in line with national, local and specialist guidelines. Maintain and promote awareness of current developments and their implications for the Medicines Management Service.

Maintain a professional portfolio for CPD recording learning outcomes through participation in internal and external development opportunities.

Responsibility for Patient/client Care


Use highly specialist clinical knowledge to work across Health and Social Care to improve medicines management, promote independence and improve care outcomes for individuals requiring support in their own homes.


Undertake individual assessments of service users and, building on the person's strengths, identify needs with respect to their medicines, optimizing independence where possible.

Ensure effective liaison with colleagues across health and social care to establish a suitable level of assistance, supporting more generic staff in the delivery of this assistance.


Demonstrate specialist knowledge to provide individual support for service users with complex needs including medication review and suggestions for alteration of medicines regime in collaboration with the service user's doctor where appropriate.
