The main objective of the post

  • To provide high-quality person-centered support to individuals within the hostel in accordance with Revolving Doors aims and principles, Code of Conduct for Healthcare Support Workers, Adult Social Care Workers in England, and other related Legislative.

Responsibilities in relation to Tenant's Support

  • Complete client's induction, including signed License Agreement, Risk and Needs Assessments, Support plan including Personal Housing Plan (PHP) on arrival in a dignifying manner, and ensure person-centered and outcome focussed support plans detailing
    Safe Exit Options in accordance with
    RDHT policy within 72 hours or arrival.
  • Residents to be made aware that this is a short-term and transitionary accommodation during assessments, and support them to make realistic choices, taking into account their views and aspirations.
  • Support residents to claim Housing benefits against their accommodation on the day of their arrival. Notify your line manager if a resident falls within "no recourse to public funds" (NRPF). Support in claiming other benefits such as Universal Credit etc.
  • Ensure residents are being supported with their health and wellbeing by effectively arranging access to appropriate health services nearby such as GP, Pharmacy, Dentist, Optician, and Mental health support and assessment pathway as at when needed.

General responsibilities

  • To communicate effectively, and to work positively and cooperatively with managers, other employees, workers from other agencies, and members of the public, and to keep supported individuals as the main focus of your efforts.
  • To take your personal and professional development seriously, at work this will include your active involvement in supervision, and also (where identified as appropriate) taking part in meetings, forums, and training.
  • To maintain records in a wide range of areas (documentation is an integral part of this job).
  • To understand issues of confidentiality in relation to your work and individuals who are supported and to ensure this is maintained.
  • To undertake other duties within the main objectives of the post as requested by people you support, colleagues, or by your line manager.

Essential Skills:
IT and Communication Skills.

Qualification:
NVQ Level 2, Level 3 is desirable
