An exciting opportunity has arisen for a full-time Adult Social Care Worker to join the friendly, supportive and well-established Hospital Discharge Assessment Team (HDAT), supporting residents of North Somerset upon leaving hospital.

About the role

Our team (HDAT) provides adult social care support, in line with the Care Act 2014 and with regard to the Mental Capacity Act 2005, to residents of North Somerset who may have care and support needs following a stay in hospital. We work alongside partner agencies within the Discharge to Assess (D2A) model.

The team consists of Senior Social Workers, Social Workers, Occupational Therapists and Adult Social Care Workers. Staff undertake Care Act assessments, mental capacity and best interest assessments, identify eligible needs and produce care and support plans to ensure a person's needs are met effectively for the longer term. Staff advocate for the individual whilst working positively with family members and carers, together with other professionals involved with the person, for the best outcomes. Staff work proactively in a strengths-based way to ensure people are also able to draw on their own abilities and support networks for a quality of life, in addition to care provision.

Our team follows the flexible worker model, which involves a mixture of time spent working at home and in our offices in Clevedon. Most of the assessment work is undertaken in the community, in care homes or people's own homes.

HDAT also has a presence in the Transfer of Care Hubs in our main local hospitals, where engaging with the person earlier in their hospital journey can minimise the disruption to their normal life and shorten their hospital stay.

This position is exempt under the provisions of the Rehabilitation of Offenders Act (Exceptions) Order 1975 and is subject to an enhanced Disclosure and Barring Service check.

A valid driving licence and own transport, or be able to provide alternative, suitable method of travel throughout North Somerset is an essential condition for this role.

Please note that while our current office base is in Clevedon, plans are in place for this to move to the Town Hall in Weston-super-Mare later this year.

About you

You should have established community experience and a good understanding of a wide range of services available throughout North Somerset. You would be working alongside professionals in our partner teams including Social Workers, Occupational Therapists, Physiotherapists and Nurses. An understanding of the Assessment Process, Personalisation and community services (voluntary, independent, Health and Social Care) would be an advantage. You would be expected to keep accurate records, and good computer skills are essential.
