Bournemouth East CollaborativePrimary Care Network**Bournemouth East Collaborative Primary Care Network (PCN) Ltd. comprises four like-minded practices working together in East Bournemouth, Dorset. The PCN serves a population of approximately 53,000 patients with a diverse demographic.

Practices part of the PCN are:

  • Shelley Manor Medical Centre and Holdenhurst Road
  • Littledown Surgery
  • Beaufort Road Surgery
  • Southbourne Surgery
The PCN is committed to developing, supporting and sustaining a diverse workforce, representative of the community it serves.

By working together with our different Network teams, we use our combined skills to provide a service that is joined-up, holistic, proactive and personal for the patient.


Our Network teams include:

  • Enhanced Care Visiting team for frail housebound patients and those in care homes. The team includes visiting GPs, ANPs, Paramedic, Frailty Nurses, Care Coordinators, Pharmacist.
  • BEC Urgent Care Treatment Centre working out of the Private Suite at Shelley Manor Medical Centre.
  • First Contact Physiotherapy Practitioner service.
  • Digital Care Coordinator, Digital Champions.
  • Mental Health practitioners.
  • Health & Wellbeing Coaches within Help & Care team.


We are recruiting for a
Digital & Transformation Lead, working 19 hours per week which can be worked on a flexible basis.

The role will be based at Shelley Manor Medical Centre, Bournemouth, located within wide green area of Shelley Park and in easy walking distance of the sea

Objective of the role (this list is not exhaustive):

The objective of the Digital & Transformation Lead role is to lead, develop and deliver a strategic transformation plan for their PCN(s).


The post holder will:

  • Lead the planning and prioritisation of digital changes
  • Work alongside practices to understand current challenges and identify solutions
  • Develop and manage a Digital transformation plan for the PCN
  • Provide handson support to practices in implementing digital transformation
  • Be a conduit and connector for digital transformation within the ICS and related networks
  • Support staff to identify and participate in relevant training and/or professional development opportunities
  • Support practices to optimise the use of digital solutions, technology, and new ways of working
  • Advise on innovative opportunities and support practices to maximise benefits from innovation, through the sharing of learning and best practice
  • Develop and implement mechanisms for assessing the impact of agreed changes
  • Provide feedback to PCN leadership and member practices on progress
  • Review and improve the PCN's digital maturity
  • Facilitate collaboration between practices and the wider system to support the delivery of care to patients
  • Use data, and improve data quality, to underpin transformation and improvement
  • Improve adoption of new technology to enhance patient access and experience, increasing PCN productivity.
  • Responsible for a range of digital transformation initiatives for the PCN, managing priorities and interdependencies
  • Ensure that digital transformation is embedded and aligned with system and national strategies.
  • Identify opportunities for digital optimisation across the PCN

  • Educated to master's degree level or equivalent professional experience
  • Project management qualification, such as PRINCE or equivalent experience
  • Managing Successful Programmes (MSP) qualification
  • Experience/

Skills Desirable:

Experience in Digital/IM&T project management or transformation experience in a Health or Social Care Environment

  • Essential:_
  • Experience in project delivery through the entire project lifecycle from initiation to delivery
  • Experience of successfully delivering change and improvement in a patient facing environment
  • Leadership skills to influence others and initiate and implement change
  • Ability to produce professional reports
  • A proven track record of communicating and engaging with a wide range of staff and stakeholders; building trusted relationships
  • Good understanding of the Digital agenda within the NHS and Social Care Sector
  • Good understanding of current healthcare policy relating to primary care and integrated care
  • Awareness of the positive disruption that digital change can bring
