You'll be leading a team of coordinators, a housing support worker, and support workers to help people in Stoke-on-Trent and Derbyshire live independently at home.
Job requirements
- Train, develop, and manage a team of coordinators, housing support worker and support workers.
- Work closely with adult social care to receive referrals and raise any concerns or issues.
- While supporting your team, you'll also need to be hands on too by completing support plans, providing community support once every four weeks, and be on call for one in every three weekends.
What we're looking for
- Someone with a good general level of education.
- Experience of working with older people, in health and social care, or having previous experience of delivering support in people's homes.
- Experience of leading a team would also be an advantage.
- A good understanding of safeguarding best practices.
- Good ICT skills with the ability to manage and update digital case files.
- Great communication skills, to build genuine and honest connections.
- Be able to plan your time well.
- Someone who is confident and resilient and can think quickly to problem solve.
- Someone who can use their own initiative to work independently.
- Life experience that allows you to show compassion instead of judgement.
- Someone with a full UK driving licence.
- Enhanced DBS check.
