We Are Hiring a Support Coordinator at Stroke Recovery Service in Somerset

We are seeking a creative and dedicated individual with exceptional communication and organizational abilities to join our team. If you have the skills to provide Key Worker support to stroke survivors and their families, this remote working opportunity is perfect for you.


Position:
S11152 Stroke Support Coordinator (x2 posts)

Location:
Home-based in Somerset with occasional travel required for meetings

Hours:
Full-time, 35 hours per week

Salary:
Circa £26,700 per annum

Contract:
Fixed-term until 31 March 2025

About the Role:
The Support Coordinator will:
- Assist new stroke survivors and their families transitioning from hospital to community
- Offer personalized information, advice, and support
- Collaborate with healthcare professionals and the community to make a positive impact

About You:
The ideal candidate will have experience in a caring profession and setting up support groups. You must be willing to travel extensively for home visits and team meetings.
