Introduction to the Role:

An excellent opportunity has arisen to come and be part of our dynamic Physiotherapy team within Southern Health. The postholder will support an increased provision to improve flow through inpatient rehab beds and within the community.

The patient case loadwill include those with diverse and complex conditions, the postholder will provide a clinical diagnosis, development and delivery of individualised treatment programmes.


Introduction to the Trust, Ward & Team:
The Southern Health are currently looking for vibrant, enthusiastic, and dedicated individual to join their team.

The therapy department are seeking highly motivated and committed physios with excellent attention to detail and strong communication to jointhem on a long-term placement contract.


Main Duties of the Job:

  • To contribute to partnership working with partners in Primary and Social care to support the Integrated Care Team (ICT)model which identifies, and case manages those patients needing complex chronic disease management or palliative care supporting the needsof the local community_._
  • To participate in professional leadership roles for AHPs in the Community Therapy Team/ Community Hospital Team(s). Act as a clinical exemplar within the team and locality/business unit.
  • To negotiate and agree with the patient, carers and other care professionals, individual roles and responsibilities with actions to be taken and outcomes to be achieved, referring on to other services or professionals as appropriate.
  • Work with Learning and Organisational Development team to ensure postregistration training needs and opportunities are communicated within the Trust.
  • To provide specialised education and support regarding therapeutic matters to the wider multidisciplinary team, including social care providers.
  • Act as clinical educator for therapy undergraduate students, maintaining good links with universities_._
  • HCPC registration
  • Registered Allied Health Professional, educated to Degree level or Diploma equivalent
  • Previous NHS experience
  • Relevant specialist/clinical experience and competence including: complex therapy/care coordination/manual handling and posture management
  • Proactively manage complex conditions
  • Practices in accordance with the professional, ethical, and legal codes of the Health & Care Professions Council
- (HCPC), and Trust and NHS protocols and guidelines
