Complex Needs Navigator

You will join a multi-agency partnership team supporting those experiencing rough sleeping or insecure housing in the district in order to increase engagement with support services and navigate a way towards settled accommodation.
Responsibilities:
- Develop relationships and trust with people experiencing rough sleeping and be a continued point of contact working closely with the outreach teams through to settled accommodation.
- Complete joint early morning outreach
- Have people on your caseload that will be reliant on you for their care and/or welfare.
- Provide support that builds on people's strengths (assetbased approach).
- Spend time supporting those that are rough sleeping to increase the likelihood of engagement in local services to support people with their recovery.
- Provide support in key locations such as City Centre, parks, prisons and hospital settings as part of discharge and release arrangements.
- Be part of a multidisciplinary partnership team and be able to call on flexible responses from other services to support individuals.
- Support individuals into temporary accommodation.
- Play a significant role in ensuring that individuals can transition to the most appropriate supported accommodation in the community.
- Identify and report back to the Homelessness Partnership any changes in services in the wider system which are needed to improve support given to the target cohort of customers.
- Ensure you operate within GDPR guidelines by regularly reviewing data held and destroying information in line with retention schedules.
