Umbrella/Ltd Pay Rate £32.00 per hour
+ Retention Bonus - £1500.00

The purpose of this role is for the post holder to be a part of Adult Services, where they will evaluate the social care requirements of individuals according to the wellbeing principles of the Care Act. The individual will employ a personalised and strengths-based approach. They will also be responsible for managing a case load, conducting thorough assessments in partnership with service users, their carers, and multi-disciplinary partners when required. The individual will clearly define the outcomes that need to be attained through this service and assist people in identifying how to meet their long-term needs and aspirations by setting appropriate asset-based goals and creating outcome-focused support plans.

Requirements
Knowledge of Adult Social Care
  • Excellent ability to demonstrate comprehension of the Care Act, as well as The Mental Capacity Act /DOLS.
  • Excellent working knowledge of the London Multi-Agency Adult Safeguarding policy and procedures, and all related adult legislation, including Making Safeguarding Personal.
  • Excellent understanding of current best practices and integration initiatives across health and care areas.
  • Excellent knowledge and understanding of the needs of Adults at Risk and those with Long-term conditions and complex needs.
  • Excellent understanding of the issues related to the delivery of assessment and support planning for people in diverse community settings, and the range of solutions available to facilitate their independence from a strengths-based perspective.



Must haves:
  • Must have a minimum of 2 years UK experience working as a qualified social worker
  • Post-qualified experience (min 2- years)
  • Social Work England registration
  • 3. HCPC registration
  • Eligible to work in the UK
  • UK state registered degree in Social Work, DipSW or CQSW.
