Job summary

Central London Community Healthcare NHS Trust are working in collaboration with East Merton PCN to offer an exciting opportunity for maternity cover for a clinical pharmacist within an established team. They will work alongside our existing CLCH pharmacy team in delivering the pharmacy PCN service.

Key components of the post include: Providing specialist expertise in the use of medicines, conducting structured medication reviews for patients with inappropriate polypharmacy and overprescribing, developing and implementing long term condition prescribing clinics and acting as a source of pharmaceutical expertise to various health and social care professionals GPs, district nurses, carers, families, community pharmacists and care coordinators.

The pharmacist will also provide leadership on streamlining services medicines audits, policies, training from the GP practice level across the PCN to support collaborative working and address unwarranted variation.

Trust wide Medicines Management Service Provision including policy development, implementation of patient safety initiatives drug alerts, patient safety alerts, training for health professionals in the community across the trust and PCN.

The post will be working in GP practices within East Merton PCN and will involve lone-working and travelling between sites.

Main duties of the job

The postholder must have relevant post registration experience in hospital pharmacy, community pharmacy or GP practices and a postgraduate qualification to at least diploma level or working towards or completed the CPPE Primary Care Pharmacy Education Pathway as well as independent prescribing qualification or currently working towards completion of this. A sound knowledge of prescribing and medicines management issues faced in primary care is essential with excellent decision-making and problem solving skills. In addition a wide exposure to and well-rounded knowledge of various clinical conditions and their management. Additionally, compassion, flexibility, excellent communication and time management skills are essential and the ability to build relationships with various health and social care professionals GPs, district nurses, carers, family, community pharmacists and care coordinators.

Job description

Job responsibilities

To provide clinical pharmacist support to East Merton PCN within Central London Community Healthcare NHS Trust (CLCH). To provide pharmaceutical support to health care professionals (HCPs) and patients within the PCN. To conduct face-to -face or remote structured medication reviews for patients in the clinic, practice setting, patients homes, care homes, bedded unit. Support long term chronic disease management through polypharmacy review or specialist medicines optimisation prescribing clinics based on needs of local population. Utilise clinical tools to proactively identify patients for structured medication reviews frail, recent discharge, history of fall Support achievement of national and local incentives. To respond to patient medication queries regarding correct use of medications, side effects or other medication-related issues involving repeat prescriptions. Support cost effective, evidence-based prescribing in line with local and national guidelines. Support and improve prescribing and medication-related processes at a practice level. Work to reduce risk of medication incidents through review of local processes and systems around medicines management, supporting high-risk drug monitoring and improving prescribing safety. To build relationships with key stakeholders within the PCN, Integrated Care System (ICS) and CLCH To ensure patients are supported to get the best out of their medicines in liaison with the persons GP, hospital clinicians, district nurses, carers, family, community pharmacists and care coordinators to ensure a multidisciplinary and integrated approach to medicines management, with a view to ensuring continuity of pharmaceutical care for patients in different healthcare settings. To provide professional support and compose patient agreed medication care plans and communicate these with other health and social care professionals Community Pharmacists. Contribute to agreed meetings within and outside the organisation and represent pharmacy as appropriate. Respond to unpredictable work patterns and urgent requests for help by phone or email within a reasonable time frame. Plan and coordinate projects, quality improvements programmes or audits within the Pharmacy Primary Care Networks team Collect appropriate quantifiable data to evaluate the service and identify areas for improvement and development

Education/Qualification

Essential

Undergraduate Degree in Pharmacy Professional registration with the General Pharmaceutical Council (GPhC) Completion of CPPE Primary Care Pharmacy Education Pathway (or working towards) Independent prescribing qualification (or working towards) Takes personal responsibility for Continuing Professional Development as evidenced by portfolio Uses reflective practice for personal and professional development

Experience

Post-qualification experience of clinical pharmacy, medicines information and prescribing advice Primary care experience including Community Services Experience in supervising a team/service and line management
