These positions are designed to contribute to avoiding hospital admission, facilitating early discharge from hospital and maintaining patients in their own homes.

Clients accessing this service will be predominantly (but not exclusively) in the elderly care group (over 65).

This post offers the opportunity to work closely with all members of the MDT both in primary, secondary care, Adult Services as well as working closely with care/nursing home staff.


The roles are about assessing and reviewing Patients re-enablement needs, it's about bringing medical and non-medical professions together under one banner to provide proactive holistic care.

The successful applicant will be proficient, knowledgeable and competent.

They will continue to develop in their role through the acquisition of further skills, knowledge and competencies, to be determined by the needs of patients, the service and the team.
