Hywel Dda Health Board present an exciting opportunity for a motivated, driven and passionate Physiotherapist to join our multi-disciplinary front of house team in Carmarthenshire as a TOCALS (Transfer of Care Advice and Liaison Service) Physiotherapist. TOCALS support a variety of patients, presentations and pathologies, including those experiencing falls and frailty, neurological conditions, cardio respiratory conditions and trauma and orthopaedic problems (including major trauma patients). As a service we support prevention of unnecessary admission into an acute hospital, to avoid the risks associated with this. Our drive is to achieve person centred care, which considers the patient holistically, promoting the person's independence and wellbeing. We play a pivotal role in promoting excellent links between acute, intermediate, community and third sector services this is vital to ensure continuity of care, and promote a right care, in the right place, at the first time approach

Main duties of the job The post holder will require a broad array of clinical experience due to the diverse population who present to front of house services. They will need to be able to provide specialist assessments with a focus upon discharge planning, from which they can develop and implement co-produced, personalised plans for patients. The post holder will play a key role in following the patients' journeys through hospital, and out into the community. They will also cover wider inpatient areas, depending on service needs.

The post holder will require excellent communication skills to work alongside the multidisciplinary team, and a wide array of fellow services who we work alongside.

The ability to work in high pressure and fast pace environments is paramount for this role, and being able to manage a clinical caseload alongside monitoring the safety of more junior members of the Physiotherapy Team.

The post holder will contribute actively to service development which is necessary to ensure the service can continue to meet the ever changing needs of the population who we support.

The role will include the opportunity to provide supervision and teaching to support workers, university students, and possibly Band 5s. Four main hospitals: Bronglais General in Aberystwyth, Glangwili General in Carmarthen, Prince Philip in Llanelli and Withybush General in Haverfordwest

Seven community hospitals: Amman Valley and Llandovery in Carmarthenshire Tregaron, Aberaeron and Cardigan in Ceredigion and Tenby and South Pembrokeshire Hospital Health and Social Care Resource Centre in Pembrokeshire

48 general practices (four of which are managed practices), 47 dental practices (including three orthodontic), 99 community pharmacies, 44 general ophthalmic practices (43 providing Eye Health Examination Wales and 34 low vision services) and 17 domiciliary only providers and 11 health centres

Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales Job description Job responsibilities The post holder will be required to participate in the provision of a flexible 7-day service, based on service need and rota basis. They will have the skills required to participate in the respiratory On-call service.

The ability to travel to location in the community is required to support the out-reach services provided by TOCALS. Cross site cover may be required (Prince Philip Hospital).
