Here at Discharge Team we believe that no-one should stay in hospital longer than their acute needs last and every patient is supported to leave hospital as soon as the first opportunity arises. Our team work on the principle of home is always the first option and we pride ourselves of working towards this ethos however not all patients are suitable for home discharge and this is where we at the Integrated Discharge Team come into our own.

If you are an enthusiastic, experienced registered professional with specialist knowledge and experience and are looking to further develop skills in discharge assessment & planning then we have the role for you in our Integrated Discharge Service.

Main duties of the job

The operational manager will have responsibility for the effective coordination of patient discharges throughout the Trust. You will be part of the leadership team within the integrated discharge service. They will play a key role ensuring that wards comply and embed the standards and processes in place, utilising 'estimated date of discharge' and 'criteria to reside' to support safe and timely discharges across the 7-day week. They will ensure 'Home First' principles are adopted, ensuring patients are assessed and supported to maximise their independence.

You will be highly organised and reliable to ensure effective functioning of this service. The service is an essential component to support management and detailed understanding of the Trust & wider health economy's capacity. Be responsible for completing department sickness reviews, manage annual leave, staff appraisals, performance reviews and will support the operational lead for discharge to deliver within agreed budgets.

As part of the role, the successful candidate will be required to work on either Trust site. You will be expected to work during the service operational hours of Monday to Friday 8am to 6pm and 8am to 4pm at the weekends/bank holidays.

Responsible for ensuring that all patients are assessed & discharged in a safe, effective & timely manner, using Home First principles Responsible for monitoring & reporting mechanisms to evidence national & local standards are met and support effectiveness of the service To ensure discharge planning and pathways are in place in the Trust and are systematically followed. To actively and effectively support the existing IDS Management team providing visible clinical and operational leadership of the team To be an effective leader & role model of the Integrated Discharge Service

Person Specification

Qualifications and Education

Essential

Educated to degree level / Management qualification or equivalent experience in health or social care setting.

Desirable

NVQ 4 in relevant subject Supervisory or management qualification Project Management qualification

Knowledge and Experiance

Essential

Good, working knowledge of NHS systems, process, data definitions and standards. Knowledge and understanding of Health and Social care act. Substantial experience in a supervisory/people management experience. Experience of working as part of a multi-agency team to deliver performance targets. Experience of delivering to performance management targets and tight deadlines. Substantial experience working in discharge or similar role.

Desirable

Knowledge of HR policies and protocols Experience in use of continuous improvement methodology Experience of management in an NHS acute setting Experience of working across organisational boundaries
