The organisation and delivery of primary care services in ELFT is being re-visioned to reflect an aspiration to deliver services in a more integrated manner in collaboration with internal and external partners.

To this end the portfolio of primary care services provided directly by the Trust is expanding and the model of service delivery is changing to include more direct participation of service users.

Directly provided primary care services range from specialist homeless practices to mainstream general practices and are located across a wide and diverse geography including central London, Bedfordshire and Luton.


  • Coordinating outreach from the practice, to include seeing, assessing and diagnosing patients that request an acute home visit. This involves prescribing or changing medication and onward referral to secondary care or other agencies as required.
  • To identify and undertake specialist tasks, to include minor illness consultations within the surgery as required
  • Recognise and work within own competence and professional code of conduct as regulated by the HCPC)
  • Providecare for patients presenting in the minor illness clinic and those that are housebound with minor illnesses and urgent on the day appointments.
  • Provide a first point of contact within the Practice for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problemsolving and clinical decisionmaking, to establish a diagnosis and management plan.
  • Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures
  • Make professionally autonomous decisions
  • Instigate necessary invasive and noninvasive diagnostic tests or investigations and interpret findings/reports.
  • Provide safe, evidencebased, costeffective, individualised patient care
  • Refer to the duty doctor as necessary
  • Refer patients to other agencies, secondary care as appropriate and complete the referral paperwork accurately, liaising with secretarial team if required

Manage, interpret and act on blood and other diagnostic test results onto the clinical system.


As a member of the clinical team, to be responsible for providing high quality care and services, including minor illness, acute care, chronic disease management, and health promotion to the practice population within the surgery (across both sites) and to those that are housebound.

To work as an autonomous practitioner providing mentorship as required.
