Key Responsibilities
  • Conduct assessments to inform treatment plans for service users.
  • Deliver tailored interventions to meet individual needs, encompassing both one-on-one and group sessions.
  • Collaborate with external agencies to address client requirements comprehensively.
  • Coordinate healthcare efforts to oversee client well-being and mitigate risks.
  • Contribute to the assessment and care planning processes, ensuring holistic support for service users.
  • Motivate clients to engage with the services outlined in their care plans.
Requirements
  • Prior experience in substance misuse services.
  • Proficiency in conducting assessments, risk management, and developing SMART care plans.
  • Experience in providing structured interventions to service users.
  • Utilization of motivational interviewing techniques in various settings.
  • Strong IT competencies.
  • Understanding of continuity of care, holistic needs, and community services.
  • Promotion of Equality and Diversity.
  • Familiarity with issues affecting substance misusers, the Recovery agenda, and Safeguarding practices.
