About the Role :
- Adult Social Care has evolved with a focus on Prevention, Early Intervention, Enablement, and Safeguarding.
- Social workers manage a diverse caseload, undertaking assessments and reviews of commissioned services, providing purposeful case management interventions to support engagement with individuals.
- Work collaboratively with partners and stakeholders to proactively respond to service demands within the financial constraints of the Council.

Responsibilities:
- Assess social care needs and undertake safeguarding inquiries.
- Develop support plans for individuals with eligible social care needs.
- Build effective relationships with internal and external partners.
- Conduct triage and plan responses for safeguarding vulnerable individuals.
- Influence planning and decision making linked to service functions.

Requirements:
- Degree in Social Work and current Social Work England registration.
- Continuing professional development and practice in accordance with professional standards.
- Experience in Local Authority social care settings.
- Commitment to social justice, self-awareness, and clear communication.
