Citycare's Health Reablement and urgent care services deal with a wide range of cases, providing care and rehabilitation for people presenting with complex pathologies, as well as challenging medical and social circumstances.
We are seeking staff with a wide range of holistic health and social care skills and experiences to compliment our service.
Visiting patients within their own homes, the teams are dedicated to preventing unnecessary hospital admissions and promoting independence and quality of life.
You will be working closely with Occupational therapists, Physiotherapists and Nurses as well as GPs, District Nurses, Community Matrons, the acute trust and a range of Health and Social Care Professionals.
