Responsible for ensuring the Emergency Department (ED) clinical system (Symphony) is accurate and up to date at all times, including patient location and waiting times.

Continually monitor the Symphony system and ensure patients are tracked against all elements of the Urgent and Emergency Care (UEC) Standards, escalating potential breaches of all UEC Standards to the Nurse in Charge and/or Consultant in charge of the department.

Key UEC standards that are applicable are as follows:
-4 hours in department12 hours from RTA (Ready to Admit) timeTime to see a ClinicianTime to initial assessmentAmbulance Handover Times Support the nurse in charge of department to maintain effective patient flows within all Clinical areas (majors, minors resuscitation, red chair assessment, ambulance handover) Maintain a live overview of the number of patients registered in each area of the Emergency Department. Pro-active in alerting the senior nurse and consultant in charge to building workload as determined by in-flow and volume of patients in each area of the ED.


Supports the team to maintain effective utilisation of available physical capacity within the department Ensures timely coding of patients leaving department to ensure accurate clock stop against the 4 hours standard Communication and team working Work effectively as part of the multi-disciplinary team.

Maintain effective communications with management colleagues regarding patient flow and performance within the department Maintains continuous communication with Clinical Site Managers and Patient Flow Matrons regarding bed availability.

Ensure that all IT faults and defects are reported to IT and followed-up in a timely manner.

Accepts and undertakes other communication requests from Senior Clinical teams that contribute to the smooth running of the department e.g.

Speciality Medical Teams.

Pro-actively pursues delays of diagnostic tests or patient movement with the following departments: - Laboratory Radiology Speciality Teams Portering Service YAS (Yorkshire Ambulance Service) Clinical Site Managers When dealing with enquiries from relatives, the post-holder will provide non-clinical information to patients and relatives about reasons for delay or changes in their pathway whilst maintaining confidentiality.

Under the guidance of the Nurse in Charge, work to safely ensure the timely discharge of patients via communication with relevant colleagues & clinical teams.

Communicate with the ambulance service, Age UK and other relevant teams to book transport for patients to aid discharge/admission, and escalate any associated delays to the Nurse in Charge.


Work with Consultant and Nurse in Charge to maintain accurate Decision to Admit (DTA) and Ready to Admit (RTA) requests on the clinical system (Symphony).

Work with the Nurse and Consultant in charge to escalate relevant issues to Hospital Security and West Yorkshire Police as needed.

Work with Portering team to ensure an adequate number of porters are available at all times to support timely transfer of patients to the wards.

Administrative Duties Maintain accurate patients records on the ED Symphony database and all associated attendance data.

Update and revise information where necessary.

Record and track the patients treatment on to the ED Symphony database, ensuring that at all time entries and relevant data items are accurately entered.

Record any test results and communications in patients ED Symphony record, ensuring nurse in charge and consultant in charge are made aware of any abnormalities.

Maintain patients confidentiality at all times.


Ensure that patient notes are scanned onto the System as soon as the patient is ready to admit to the ward, checking that all required transfer documentation is present prior to scanning.

Ensure patient location audits are carried out every 4 hours, and are fully reflective of the Symphony database, escalating any discrepancies to the Nurse in Charge, and updating the Symphony as appropriate.
