Job summary

To lead on the delivery of the 0-19 Healthy Child Programme and Public Health Nursing Programme in line with local and national public health priorities to children, young people and families who may be new to the country, young people at risk of sexual exploitation, parents with mental ill health, drug, alcohol and/or domestic violence that are often living in temporary accommodation/refuge and find it difficult or reluctant to access existing services.

The post holder will have the knowledge and skills to identify risk factors and hold a caseload of families that are complex in nature. They will work in partnership with families who may be new to the country, young people at risk of sexual exploitation, parents with mental ill health, drug, alcohol and/or domestic violence that are often living in temporary accommodation/refuge and find it difficult or reluctant to access existing services.

The post holder will have responsibility for identifying transient families, completing health needs assessments, co-ordinating health plans and additional targeted support with the Healthy Families Teams to improve outcomes for some of the most vulnerable young people and families in the local population.

Main duties of the job

Ashfield and Mansfield Healthy Families Team, provide a quality service to the local population, we are an integrated service, working in partnership with local Children's Centres, working with children, young people aged and their families.

This is a specialist post within the Healthy Family Team. The post holder will have responsibility for identifying transient families, completing health needs assessments, co-ordinating health plans and additional targeted support with the Healthy Families Teams to improve outcomes for some of the most vulnerable children, young people and families in the local population.

A key aspect of the role will be developing links and building supportive relationships within primary care, relevant statutory and voluntary agencies. The post holder will be a source of expert knowledge and support for health colleagues and work collaboratively with partner agencies.

A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

Essential

First level registration with NMC Registered Specialist Practitioner (Health Visitor / School Nurse) Non Medical Prescriber V100

Desirable

Recognised teaching qualification (EMB998/730/FLIPS) or equivalent Evidence of further professional development Independent prescribing V150

Experience

Essential

Demonstrate Health Visiting / School Nursing practice Experience Experience of working in a multi-disciplinary team Experience of supervising junior staff Ability to work in partnership with other services to support children & families
