T he post holder will work as part of the pharmacy and medicines optimisation team, ensuring that all medicines ordered, prescribed and supplied within the locality is carried out in a safe and cost-effective manner. The post holder will work closely with the Locality Care Coordination Pharmacy Technician and Care Co-ordination team, to provide a single joined-up service for people needing health and social care support. To provide specialist clinical pharmacy in a specific area of clinical and/or operational practice, to support the medicines-related care of patients, support the provision and development of medicines management services and to participate in training, education and research as required. 1. To actively contribute to, and participate in, the provision and development of pharmaceutical services, and safe and effective use of medicines, within the South Essex Partnership University NHS Foundation Trust, as part of the Trust's pharmacy and medicines management team. 2. To participate as an active member of specific ward / unit / clinical teams, as required, and support the safe and effective use of medicines. 3. To provide clinical pharmacy and medicines management advice in order to maximise benefit and minimise risk in relation to medicines 4. To support and participate in clinical audit.

Main duties of the job Are you a qualified and experienced registered pharmacist looking for an exciting new challenge? We have an opportunity for an ambitious and forward thinking pharmacist to join the Care Coordination Community Health Services Team as an integral member of the Essex Partnership University Trust (EPUT) Pharmacy Service.

We are seeking an experienced, enthusiastic, self-motivated pharmacist with excellent communication skills and a clinical background to work within South East Essex integrated health services

The post involves working alongside a multidisciplinary team of healthcare professionals to provide an integrated healthcare service to the community. You will demonstrate a high level of clinical expertise and participate in delivering medicines reconciliation, medication reviews, audits, and other medicines optimisation initiatives.

You will be expected to work independently and show initiative, reporting to the Lead Pharmacist, Community Health Services (South East Essex) and may be required to attend meetings representing the Lead Pharmacist. Job description Job responsibilities KEY RESPONSIBILITIES Clinical Practice - Conduct clinical medication reviews for patients identified as requiring support- Make appropriate prescribing recommendations and communicate decisions to all stakeholders- Ensure safe and appropriate use of medicines in order to maximise benefits and minimise risk to patients. This will include regular review of prescriptions, identification of drug interactions and adverse effects, advice on clinical monitoring, compliance with legislation (for example, Medicines Act, Mental Health Act), compliance with Trust policies and guidelines.- Provide a clinical pharmacy service to Care Coordination team and assigned wards, and to the multidisciplinary team caring for patients.

- Take an active role in the pharmacy and medicines management team and provide support to less experienced staff providing a service to the Trust, acting as a clinical role model.- Seek advice from senior pharmacy colleagues in order to provide advice to clinicians on unlicensed medicines use and areas of practice where the evidence base is poor.- To work as part of a multidisciplinary team to provide safe, clinically effective and cost efficient use of medicines in patients, including participation in consultant ward rounds, and multidisciplinary team meetings. Provide highly specific advice to clinical team members on the management of complex medicines related issues, such as complex drug interactions, medicines use in pregnancy, unlicensed use of medicines etc.- Make appropriate referrals to the Lead Pharmacist SEECHS and other members of the multidisciplinary team and within the pharmacy team.- Advise medical, dental and nursing staff, and allied healthcare professionals on the actions, properties and the safe, cost effective use of drugs, and be able to critically appraise the literature and give accurate interpretation in order to improve use of medicines. Utilise medicines information resources to further clinical pharmacy practice in community health services- Provide advice which is accurate, evaluated, impartial and up-to-date to ensure the safe, clinical effective and cost efficient use of medicines.- To work as an independent non-medical prescriber, within the designated areas of practice (following training and a period of actively prescribing as a supplementary prescriber if required), to work Responsible and accountable for the assessment of patients with undiagnosed conditions and for decisions about the clinical management required, including prescribing

Community pharmacist applicants are welcome, prior hospital experience not required. Person Specification Qualifications Essential
  • Masters Degree in Pharmacy or equivalent acceptable for registration by the GPhC
  • Current and continued registration with the GPhC as a practicing member
  • Postgraduate Certificate or Diploma in relevant area of pharmacy practice, or equivalent experience of at least two years post registration
  • Member of the Royal Pharmaceutical Society and willing to work towards Foundation or Faculty membership
Desirable
  • Certificate in Management studies or equivalent experience
  • Qualification as an Independent Non-Medical Prescriber/currently attending or prepared to commence appropriate course to qualify as an independent non-medical prescriber
Knowledge Essential
  • Evidence of Continuing Professional Development
  • Broad technical, clinical and pharmaceutical knowledge relating to the promotion of high quality, effective and cost efficient use of medicines
Skills Essential
  • Literate in IT/Computer Skills
  • Excellent interpersonal, influencing and negotiating skills including effective written and verbal communication skills
  • Good organisational skills e.g. prioritising and time management, and able to work as part of a team or alone
  • Ability to work under pressure and demonstrate good time management
  • Relevant post-registration clinical pharmacy experience in an acute, primary care or care-home setting
