We are looking for flexible, dynamic and enthusiastic Band 4 Assistant Practitioners to join ournew and exciting Urgent Community Response Team, covering the South Warwickshire locality.
This is a new service which has been developed using the Aging Well Programme.This distinguished service model will focus on more care in the community, better use of technology and increased multi-disciplinary working.
Key elements will include responses in 2 hours, with short term intervention for up to 48 hours.The post is a community post and the ability to meet the travel requirements of the role is essential.

Ideally you will have experience of working in a community setting however, if you don't have this experience but really want to work in the community we still want to hear from you but you must have NHS and UK experience.
The service operates from 8.00am until 22.00pm 7 days a week. Applicants will also be expected to work weekends and bank holidays on a rota basis. Twilight shifts available such as 16:00pm - 22:00pm.The role of the Assistant Practitioner is to deliver a high quality, caring and compassionate patient care.
The post holder will be undertake assessment, implementation and evaluation of patient care needs for a defined group of patients under the indirect supervision of a registered professional in their specialist area.
The post holders are required to work as part of a team committed to delivering and improving a high standard of nursing care within South Warwickshire NHS Foundation Trust.
