Intervention & Transition Team - Fixed Term

Worcester, Worcestershire, United Kingdom Maggs Day Centre Full time

Our Intervention and Transition Workers (ITT Team) work with people experiencing homelessness from initial contact until they are no longer needed to.

Typically, our
ITT team works with the most complex, hard to reach entrenched rough sleepers who have previously refused any other support or have felt let down by previous agencies.

Therefore, our team will develop a single, consistent, and trusted point of contact with the individual being supported and work in a way that makes safeguarding personal - hear the person, respect the person, and be honest with the person.

Main Purpose of Job:

  • To end the cycle of homelessness and rough sleeping in Worcester City Centre. Your role is to "engage the unengaged" which means you need to be patient, resilient and adapt well to ever changing situations. Your role is very varied as you cannot predict who or what you will see on a daily basis, keeping a level head is a must.
  • Working in line with Maggs procedures to support People experiencing Homelessness in working towards meeting their goal. You should work hard to recognize and understand what goals the team and service users are working towards; we do not operate "a one size fits all" approach and this needs to be reflected in the way you work.
  • Working collaboratively with relevant agencies to deliver the best quality of service for the client tailored to the individual's needs. You will be liaising with a range of other services on behalf of our service users including Housing Associations, Benefits Agencies, Medical Agencies, and other organisations dedicated to helping rough sleepers in Worcestershire. You need to feel confident advocating for the people we work with and building relationships with other agencies to ensure we can achieve the goals that our service users have set.
  • Staff must be approachable, friendly, and helpful and work well within a small team. You should have a 'can do' attitude, be reliable and flexible and 'pull your weight'. In addition, you should understand the importance communication plays to work effectively in a team of people irrespective of whether they are paid or volunteers.

Detailed Duties and Responsibilities

  • To liaise with other services and agencies to improve access for Service Users, individually or as a group. To develop and maintain positive professional relationships with these agencies. To promote positive relationships with the local community and businesses, including dealing sensitively and professionally with any complaints and attend multiagency meetings as required.
  • To complete all relevant paperwork with all service users and to work with service user to progress. All our support packages need be tailored to individual needs. Ensure that record keeping is accurate and up to date, this includes documenting all work done and maintain records in line with Data Protection and Confidentiality policies and procedures.
  • To carry a caseload and maintain accurate records throughout the lifetime of support; to attend case work meetings with people experiencing homelessness, other workers and professionals and to implement agreed support plans. To ensure outcomes can be monitored and reported in line with requirements.
  • The scheme is relatively small and relies on staff filling in for each other. All staff are therefore expected to carry out other duties and responsibilities, as requested, providing they do not alter the general level and character of this job description.

We warmly welcome applicants from black, minority, ethnic (BME) communities, disabled people and lesbian, gay, bisexual and trans (LGBT) communities, as they are currently under represented within our workforce - this diversity is vital to our journey and is fundamental to our values and enriches the service we offer the people we work with.
